Still at home, still largely independent, but the tablets, the appointments and the shopping have quietly become a rota. Usually run by whichever adult child lives nearest — and this is how the rest of the family finally sees what that takes.

One of you does the visits, one manages the money, one is three hours away and worried. Everybody sees the same day, so nobody has to be phoned for an update and nobody is left out of one.

A hip, a stroke, a fall, an operation. Six weeks of medication at new times, physio, district nurses and follow-ups, arriving all at once. Set it up on the ward and hand it to whoever is on tomorrow.

Living with a long-term condition
Dementia, Parkinson's, MS, cancer, frailty. Small changes matter and are easy to lose track of; a journal kept by everyone who was actually in the room is the record that shows a GP what has really been happening.

A disabled adult or child
Care split between parents, personal assistants, respite and school or day services. Everyone works from the same medication list and the same handover, whoever is on.

Covering for someone else
A sibling on holiday, a carer off sick, a fortnight of respite. Open the app and you can see what normally happens, rather than piecing it together from a note on the fridge.

Agency and council carers can be invited into the circle to log visits and see what the family has already done — without a separate system or a paper book nobody reads.
And who it isn’t for
If you are a care agency managing rounds across many clients, this is the wrong shape — it is built around one person and the people who love them, not around a workforce. And if what you need is clinical advice rather than a way to organise, you want the NHS, not us.
